o New patient intake and orientationo Conducts initial assessment of patient problems and needs and reports to clinical team to initiate treatment planningo Conducts individual sessionso Facilitates groups with patientso Addresses patient's concerns and helps eliminate possibility of AMAo Provides Case Managemento Provides tours for prospective patients and their families as requested.
o Documentation in patient medical recordCounselor Requirements:
o Must have one of the following:
